Job Description

Case Manager - Meadowview Our Case Manager is a key member of our interdisciplinary care team, which is focused on providing person-centered care that is highly individualized and attuned to the specific needs and preferences of those we serve. Day-to-day responsibilities involve resident assessments and interventions, interface with other interdisciplinary team members, supervision of the Residency Coordinator and education of residents, family members, and staff. A love for working with Seniors is a must, as is having a passion for bringing healing and hope to those we serve. Successful incumbents will possess a nurturing disposition, strong interpersonal skills, and the compassion it takes to get the job done while building strong bonds and relationships. Responsibilities Assist with pre-admission activities. Assists with new resident adjustment to the facility. Provides ongoing professional social work services to each resident for the duration of their residency. Provides oversight to the
Residency Coordinator:
Documents appropriately and in a timely manner in resident charts (which may include specific 1:1 visits, group work, mental health updates and psychosocial interactions), including a resident summary/update at least once every six months. Assists in identifying and carrying out discharge plans Participates in relevant meetings. Provides education to registrant/family/staff. Works with the psychiatrist/psychologist to insure the mental health and well-being of the residents. Maintains and develops work-related knowledge and skills through continuous learning activities, attendance at in-services and workshops, etc. Completes special projects/tasks in support of effective department functioning as assigned by supervisor. Education Bachelor's degree in social work required. Master's degree in social work is preferred. Experience 3-5 years of professional experience in a long-term care/assisted living setting preferred. Will consider exceptionally strong candidates who have a LMSW or LCSW without 3-5 years of experience.
