Job Description
A Social Worker III - Adult Services provides protective and case management services to disabled and vulnerable adults. This work includes evaluating allegations of abuse, neglect, and exploitation; providing case management services; assisting and protecting adults who were found to be incompetent and in need of Adult Guardianship services; and assessing the needs and providing services to disabled adults who wish to remain in their homes.
Job responsibilities include:
Assessing the safety of elderly and disabled adults through interviews, completing home and community visits as well as contacting family members and service providers.Connecting vulnerable adults to community resources to address safety and risk factors.
Completing timely, concise, detailed documentationPreparing court documents and testifying in court as needed.
Conducting interviews with disabled adults and others with knowledge of the adult's abilities and needsConsulting with professionals regarding the adult's history, needs, and abilitiesReviewing medical and/or psychological recordsRotating on-call coverage for adult services as assigned
Knowledge, Skills and Abilities:
Knowledge of adult services policies and legal mandates Knowledge of social work, practices, and principles Working knowledge of Microsoft Office SuiteGeneral computer knowledge and ability to learn and navigate various programs and databasesExcellent customer service skillsAbility to prioritize tasks and reassess as priorities change in a fast-paced environmentAbility to de-escalate upset clients / remain calm in stressful situationsStrong written and oral communication skillsOther Information:

Must be physically able to operate a variety of office equipment.
