Job Description

Social Worker / Social Services Professional 107 12th St S, Cooperstown, ND 58425 Full-time Full-time Position Overview We are seeking a compassionate and dedicated Social Worker / Social Services Professional to provide services to both long term care residents and hospital patients. The Social Worker will identify psychosocial, emotional, mental health, and discharge planning needs while coordinating services that promote dignity, independence, recovery, and quality of life. This position works collaboratively with residents, patients, families, interdisciplinary staff, physicians, and community resources to ensure individuals receive the highest practical level of physical, mental, and psychosocial well-being. Services are provided in accordance with the National Association of Social Workers (NASW) Code of Ethics and all applicable state and federal regulations. Major Duties and Responsibilities
  • Complete psychosocial assessments and care planning
  • Provide emotional support, counseling, and crisis intervention
  • Coordinate discharge planning and community referrals
  • Advocate for patient and resident rights
  • Collaborate with interdisciplinary teams, families, and community agencies
  • Maintain accurate documentation and regulatory compliance Qualifications
  • Current North Dakota licensure as an LSW, LCSW, or LICSW preferred OR
  • Bachelor's degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, psychology, or related field, with at least one year of supervised social work experience in a healthcare setting working with individuals
  • Experience in healthcare, hospital, or long-term care preferred
  • Strong communication, assessment, and care coordination skills We Offer
