EXCITING OPPORTUNITY TO TRULY MAKE A DIFFERENCE
We're looking for a compassionate, motivated, bilingual case manager to join our agency. Your job is to help cancer patients and their families to access medical, financial, and other resources necessary to manage their day-to-day needs during the treatment phase of their illness. Cancer Support Team is a licensed non-profit agency that assists anyone with a cancer diagnosis

Our team of registered nurses, case managers and social workers help patients cope with the cancer experience by giving them the information they need to find the best possible healthcare, treatment, resources, and social services. For nearly 50 years, we have worked one-on-one with patients at their home or in a community setting teaching and advocating for them.
Requirements:
BA in related field required. Bilingual (Spanish/English) Preferred previous experience in human services. Excellent computer skills Driver's license and vehicle Responsibilities Manage a selected caseload of patients and families in need of non-medical case management especially related to accessing entitlements and benefits on a Federal, State, and local level, such as Social Security, Medicaid and other Department of Social Services benefits. Conduct patient/family interviews, by telephone, in CST office or in patient/family residence to learn information about patient/family needs. Conduct necessary research to determine the most appropriate resources for patients and families. Assist patients in accessing entitlements, benefits and resources including assistance in completing required applications. Plan and execute patient and family education programs with CST staff. Establish and maintain effective working relationships with CST and community Agency staff and collaborate on patient care issues. Document in a timely manner, all necessary information in patient records and advise other team members of changes in patient and family status as needed. Communicate effectively with CST staff and outside agencies, both orally and in writing. Establish and maintain systems of data collection related to patient and family case management needs and services provided by CST. Discuss with supervisor patient issues and concerns to enhance skills in working with patients and families. Salary $47,000
