Speech Language Pathologist Responsibilities As a Home Health Speech Language Pathologist, you will provide skilled one-on-one speech therapy services in patients' homes, helping them improve communication, cognitive abilities, swallowing function, and daily independence. Key Responsibilities Conduct in-home speech therapy evaluations, reassessments, and follow-up visits Develop and implement individualized treatment plans based on patient needs and goals Assess and treat speech, language, cognitive, voice, and swallowing disorders Educate patients and caregivers on communication strategies, swallowing safety, and home exercise programs Complete timely and accurate documentation using WellSky Kinnser EMR Collaborate with physicians, nurses, physical therapists, occupational therapists, and interdisciplinary care teams Monitor patient progress and adjust treatment plans as appropriate QualificationsRequired Active Arizona Speech Language Pathologist (SLP) License Master's Degree in Speech-Language Pathology or related accredited program Valid driver's license, reliable transportation, and current auto insurance Strong communication, organization, and time-management skills Ability to work independently in a home health setting Preferred ASHA Certificate of Clinical Competence (CCC-SLP) CFY Candidates Welcome to Apply Current CPR Certification (AHA preferred) Minimum 1 year of Speech Therapy experience Home Health Speech Therapy experience Knowledge of Medicare home health documentation and compliance standards Experience using WellSky Kinnser EMR What We Offer Competitive Per-Visit Compensation Up to $105,300+ Annual Earning Potential $5,000 Sign-On Bonus Flexible PRN Scheduling Opportunities Consistent Patient Volume Supportive Leadership & Clinical Resources Autonomy and Work-Life Balance Opportunity to Grow Within a Brigade Health Company Apply Today If you are a Speech Language Pathologist seeking flexibility, independence, and rewarding home health patient care, apply today to join Home Sweet Home, a Brigade Health company, and help patients safely remain independent in the comfort of their homes.
