PRN Speech Language Pathologist (SLP) - InHouse Rehab | Lamesa, TX Focused Post Acute Care Partners (FPACP) is a dynamic company with an expectation of excellence in the delivery of long-term care to every patient every day. Position Summary The Speech-Language Pathologist (SLP) will evaluate, diagnose, and treat residents with speech, language, cognitive-communication, and swallowing disorders . This role includes developing individualized treatment plans, educating families and staff, and collaborating with the interdisciplinary team to support optimal resident outcomes. Key Responsibilities Evaluate residents for speech, language, cognitive, and swallowing impairments Develop and implement individualized treatment plans based on resident needs Provide skilled therapy services aligned with medically necessary goals Implement augmentative and alternative communication (AAC) strategies when appropriate Educate residents, caregivers, and staff on treatment techniques and progress Collaborate with physicians and care teams to coordinate instrumental swallow studies (e.g., MBSS, FEES) Document services accurately and in a timely manner per regulatory and facility requirements Respond to family inquiries and provide ongoing communication regarding care Support and train restorative nursing staff as needed Ensure compliance with all legal, ethical, and professional standards Maintain resident dignity, confidentiality, and quality of care at all times Qualifications Active Texas Speech-Language Pathologist license ASHA Certification (CCC-SLP) In good standing with all licensing boards and regulatory agencies Experience in long-term care, skilled nursing, or geriatric rehabilitation preferred Familiarity with Net Health documentation system a plus Strong interpersonal, communication, and clinical skills Why Join Us?
