This Role:

Communication & Cognitive Therapy:

Evaluate and treat speech, language, and cognitive-communication disorders. Support patients with memory, attention, and problem-solving challenges.

Swallowing & Safety Management:

Assess and treat swallowing disorders (dysphagia). Provide recommendations to improve safe eating and reduce aspiration risk.

Personalized Care Delivery:

Develop therapy plans tailored to each patient's home environment and goals. Modify interventions based on progress and clinical findings.

Patient & Caregiver Education:

Teach strategies that patients and caregivers can apply between visits. Promote long-term carryover of communication and swallowing techniques.

Documentation & Coordination:

Complete visit notes using a digital EMR system. Communicate with care team members as needed to support patient outcomes.

Qualifications:

Active New York State Speech-Language Pathologist license. Experience with adult populations preferred. Knowledge of dysphagia and cognitive-communication treatment. Strong interpersonal and patient education skills. Comfort working in a mobile, home-based setting.
