Job Description

Job Summary:
The spiritual care coordinator is responsible for the provision of spiritual care services to patients/families/caregivers of Pathway Hospice, either directly or through coordination of care with other spiritual counselors.
Qualifications:
  • Ordained and endorsed minister, priest, rabbi or a member of a religious order; Bachelors or Master's degree preferred in Divinity, Theology, Pastoral Ministry, or Religious Studies.
  • Have counseling experience focusing on hospice and end of life.
  • Documentation demonstrating current faith-group endorsement. Strong commitment to the value that "Palliation is a valid and appropriate choice for people at any stage of their encounter with potentially terminal disease." Strong commitment to the Pathway Mission and Vision Statement. Comfortable in an interfaith setting.
  • Ability to establish a caring and empathetic relationship with patients and families.
  • Compassion and empathy for people with end-of-life issues.
  • Knowledge and understanding of hospice philosophy and palliation.
  • Evidence of maturity and ability to deal effectively with job demands.
  • Demonstrates excellent verbal and written communication skills.
  • Owns reliable transportation, has a South Carolina Driver's license and auto liability insurance.
  • Possesses a practical understanding of diverse theological perspectives, as well as religious and cultural traditions.
  • Ability to employ self-initiative, planning and organizational skills in working with a high volume, yet ever-changing patient caseload.
  • Able to maintain high level of integrity, accountability and confidentiality in one's work as a member of an interdisciplinary team.
  • Able to serve responsibly and compassionately in a context of differing developmental and situational needs including age, race and socio-economic backgrounds.
Duties:
General Responsibilities
  • Provides spiritual support and counseling to patient and families as articulated in the plan of care.
  • Performs spiritual assessment including an assessment of the patients' spiritual preferences.
  • Participates in the re-certification process.
  • Assesses and evaluates need for changes in level of care and communicate this with IDT.
  • Develops and maintains contact with appropriate community resources, especially faith communities, in order to assure appropriate referrals.
  • Provides emotional/spiritual support to Pathway Hospice staff and other IDT members.
  • Maintains up to date patient records to ensure that problems, interventions and goals are accurate and follow the Plan of Care.
  • Completes visit notes, daily logs and time sheets in a timely manner in compliance with Pathway Hospice Policy and Procedures.
  • Writes concise easily read visit notes.
  • Coordinates IDT reflection. Manages time effectively and shows dependability on assigned job duties. Maintains satisfactory work attendance. Wears neat and professional attire including name badge during work hours.
  • Serves as a resource to the IDT.
  • Shares on call duties as requested.
  • Performs duties in compliance with Pathway Hospice Policy and Procedures.
  • Performs other duties as assigned.
Patient Rights Adheres to the privacy practices as described in the Notice of Privacy Practices and as required by the privacy rule of the administrative simplification provisions of the Health Insurance Portability and Accountability Act of 1996. Ensures that all care is provided with respect to patient rights with a strong emphasis on the following values: Patient self-determination, and autonomy, Patients and families are regarded as experts in determining how they will live and die, Respect for one another. Do good and do well. Reports all grievances and complaints made by patient, families, referral sources, or other members of the community to the appropriate persons. Practices patience in voice and manner Safety and Sanitation Reports all accidents and incidents observed. Identifies and responds appropriately to emergency situations. Observes safety precautions when in the office and out in the field. Follows universal precautions and infection control procedure according to OSHA regulations and Pathway Hospice policies. Observes safety needs of patients. Professional Development
  • Maintains current knowledge of spiritual interventions at the end of life by participating in continuing education programs, workshops, and professional organizations, along with seeking ways to improve professional skills.
Attends staff support meetings and other meetings as required. Plans and prioritizes goals. Attends all IDT meetings having less than three unexcused absences in one year.   • Conducts funeral/memorial services when appropriate and requested by patient's family.
  • Participates in bereavement follow-up as necessary.
  • Assists with the training and management of Spiritual Volunteers.
  • Maintains positive relationships with clergy and religious leaders in the community to ensure public relations and marketing needs are met.
  • Participates in orientation and training programs as requested. Thinks strategically about the future. Embraces opportunities. Participates in agency and community programs as requested.
  • Facilitates positive relations with other medical professionals in the community.
Delivery of Care Reports any changes in patient condition or family situation by notifying the interdisciplinary team. Follows the Interdisciplinary Plan of Care for each patient Provides appropriate support at the time of death.

$18.00 - $26.00 per hour
