Job Description

Responsibilities include maintaining a centralized or coordinated process designed to coordinate program participant intake assessment and provision of referrals covering a geographical area and is easily accessed by individuals and families seeking housing or services and is well advertised including a comprehensive and standardized assessment tool. Serve as the Coordinated Entry expert in the Local Homeless Coalition. Ensure a consistent and accurate flow of information between the WI BOSCOC and the Local Homeless Coalition. Run reports out of HMIS. Ensure all participating agency staff within the local homeless coalition have completed the required training. Coordinated Entry training will be documented by SSO staff and communicated with ICA in order to receive access to the BOS user in HMIS. Coordinated Entry System Specialist in order to receive access to the Non-HMIS system. Attend all Coordinated Entry and Coordinated Entry Lead training. Provides updates on Coordinated Entry system updates, changes, etc. to the local homeless coalition. Works collaboratively with the Non-HMIS List Holder (if this role is done by a different person in the local homeless coalition.) Maintain copies of all Agency Agreements and Staff Agreements for the local homeless coalition Coordinated Entry System, and ensure they are updated annually in June. The SSO staff keep the Staff Agreements on file. The Agency Agreements must be submitted to the

Coordinated Entry System Specialist. Update the Coordinated Entry-SSO Compliance Tracking form for participating agencies in the local homeless coalition Coordinated Entry System on a regular basis. Ensure participating staff and agencies comply with the Coordinated Entry Policies and Procedures manual and required appendices. Ensure marketing and outreach activities occur monthly within the local homeless coalition, as required by the
WI BOSCOC
policy. Ensure all No Wrong Door agencies Coordinated Entry marketing materials display the No Wrong Door logo and the No Wrong Door flyer is posted at the entrance of the agency, at minimum. The SSO must have a means of transportation to distribute Coordinated Entry marketing material and cover the entire geographical area of the local homeless coalition. The SSO must have a means of transportation to provide outreach in the local homeless coalition. This includes but is not limited to, traveling to homeless encampments, participating in street outreach, traveling to different community events (e.g., resource fairs, stand-downs, etc.), and traveling to unsheltered areas to connect with those not currently seeking services. Ensures the local homeless coalition Coordinated Entry System has established an after-hour plan for accessing emergency services. Ensure all relevant stakeholders are aware of the plan. Ensure the plan is updated annually, as identified by the local homeless coalition. Participate in or facilitate Coordinated Entry case conferencing monthly, at minimum, as described in the Coordinated Entry Policy & Procedure manual. This includes, but is not limited to, using a holistic multi-agency/multi-sectored approach to case conferencing. Comply with all HMIS policies and procedures. Other duties as determined by the CoC and Unit Supervisor. Bachelor's degree and two years' work experience preferred Possess good knowledge and sensitivity of the low-income community, their needs and concerns as well as resources available to assist them Working knowledge of computers including Internet navigation, Word, Excel and other database programs Must be willing to learn and utilize multiple database systems. Must have good communication and organizational skills, show the ability to exercise good judgment, and to maintain confidentiality at all times Must have the ability to work independently, exhibit self-initiative, work well with co-workers and public Must be detailed oriented and able to keep accurate records and precise reports Case Management experience a plus Requires own vehicle with insurance for job related travel
