  • MENTAL HEALTH (PSH) Support Coordinator II
  • Mental Health Encompass Community Supports, formerly known as Rappahannock Rapidan Community Services (RRCS) is hiring a Support Coordinator II
  • Mental Health.

Our Support Coordinator
  • Mental Health assesses clients' needs and provides access to needed and appropriate services including treatment, rehabilitation and service linkage, case monitoring and client advocacy functions; work is performed under regular supervision.
SOME OF THE RESPONSIBILITES
Assesses client service needs in the setting most comfortable for the client; presents options and services based upon the needs and preferences of client. Develops an individual service plan of care based on assessed needs and client consensus; reviews and updates service plans. Ensures programs operational compliance to all regulatory guidelines; Implements new programs, policy changes, and requirements as assigned, completes mandated trainings. Completes referrals and linkages to appropriate agencies for the agreed upon services. Provides linkages to transportation options and provides transportation on a limited, case-by-case need. Makes home, hospital or program visits to evaluate the quality of services provided changes in client's condition and counsels clients as necessary; evaluates client's environment for safety, security, negative factors or positive factors. Coordinates and communicates with multiple service providers and agencies; attends/facilitates inter-disciplinary team meetings to discuss decisions for client plan of care, assists with discharge planning from care facilities. Provides supports when needed to include crisis prevention services and linkages to crisis intervention services. Completes required documentation, tracks and maintains a variety of reports. Prepares and maintains client charts and other records; prepares applications and reports related to benefits. Supports clients during a variety of appointments, meetings, and court appearances; Attends meetings, staffing, and conferences as they relate to client, staff and program needs.
KNOWLEDGE, SKILLS AND ABILITIES
General knowledge of community and agency resources. General knowledge of interviewing and supportive counseling techniques. General knowledge of social casework techniques. general knowledge of human development and behavior Ability to analyze facts and exercise sound judgment. Ability to professionally communicate complex ideas, orally and in writing. Ability to establish and maintain effective working relationships. Ability to utilize electronic health records. Ability to coordinate ideas and resources to achieve goals. Knowledge of MS Office, to include Word and Excel.
REQUIRED QUALIFICATIONS
Bachelor's degree in human services or a related field. 1 year of related clinical experience providing direct behavioral health services to adults with mental illness. Valid Virginia driver's license is required. Employment with Encompass Community Supports is contingent upon the successful completion of our background check process (criminal, DMV, education, drug screening, reference check, DSS, OIG and SAM).
PRFERRED QUALIFICATIONS
Certified Qualified Mental Health Professional-Adult (QMHP-A), or QMHP Trainee. Two years of clinical experience providing behavioral health services. Spanish Speaking is a plus. Encompass Community Supports is an Equal Opportunity Employer.
