Pathfinder is seeking a Supported Living Manager (SLM-2) to oversee staffing, medical coordination, and daily operations for individuals receiving supported living services. This role ensures that individuals with intellectual and developmental disabilities receive high-quality, person-centered support in their homes and communities. The SLM-2 is responsible for scheduling, advocacy, staff supervision, and ensuring compliance with service plans and regulatory standards.Key Responsibilities:
As a Supported Living Manager (SLM-2) atPathfinder, You Will:
Manage Daily Operations & Staffing Schedule and coordinate staffing to ensure quality support. Advocate for individuals in planning meetings and financial matters. Oversee medication compliance and ensure all prescriptions are properly managed. Supervise and mentor Direct Support Professionals (DSPs) to implement person-centered services. Ensure Compliance & Documentation Maintain Person-Centered Individualized Support Plans (PCISP) and upload quarterly summaries. Oversee Medicaid, DDRS, and BQIS documentation. Work with the Call Center to ensure proper staff coverage. Participate in audits and implement corrective action plans. Train & Support DSPs Provide training in orientation, mentorship, and person-centered planning. Approve time sheets and conduct monthly staff meetings. Guide DSPs in integrating daily living tasks as learning opportunities. Coordinate Medical & Community Services Schedule and oversee medical appointments and medication distribution. Act as the primary contact for Interdisciplinary Team (IDT) members. Help individuals access community resources and build support networks. Engage in Direct Care & On-Call Support Provide hands-on support in homes as needed. Participate in an on-call rotation to address staffing and emergencies. What You Bring to theRole:

Preferred Minimum Qualifications:
Minimum of a high school diploma/GED. Prefer 2 years experience in the field of Human Services. At least 18 years of age. Basic language, written, and computer skills necessary for communication and documentation. Demonstrated ability to support people with disabilities. Ability to work a flexible schedule if needed. Crisis prevention, intervention, and teaching skills. Valid driver's license, insured vehicle, and safe driving record. Ability to speak, hear, and read the English language. Be able to run, squat, stoop/bend, kneel, climb stairs, lift up to 50 pounds, push and pull against potential resistance, and implement non-physical and approved physical interventions and CPR with people supported when needed. He/she shall have adequate vision, writing, and documentation skills to perform the essential functions of the job.
