In-Person $45000.00 - $55000.00 per Year 2 Years Experience High school graduate or equivalency certificate (GED) No Commission 40.00 hours per week / Day Shift / Full-Time / Hours May Vary Description The Supportive Housing Case Manager is responsible for providing comprehensive case management services to individuals and families participating in Opportunity House's Supportive Housing Program. This role ensures a smooth transition into stable housing and supports clients in achieving long-term stability and independence. The Case Manager also provides supportive services such as information and referral, goal setting, problem solving, and linkages to any needed wrap-around services.
ESSENTIAL DUTIES AND RESPONSIBILITIES
include the following: Participates in case reviews regularly with Housing Director and Case Management team. Processes initial case management intake of new residents. Sets appropriate goals for housing, mental health and medical needs, employment or other funding options for assigned clients. Assists residents in finding needed community resources to enhance their lives and promote independence Maintains resident files and documentation of work performed. Assist with necessary referrals and advocacy of residents such as LIHEAP, food bank, Medical Assistance. Ensures that quality housing and services are provided to residents in a manner consistent with the values and policies of Opportunity House. Teaches necessary skills on budget planning, resume writing and other life skills as needed to residents. Addresses the needs of any children in assigned cases to ensure their needs are met regarding health, education and emotional wellbeing. Interact regularly with residents, identifying needs, understanding issues and working towards positive resolutions. Reports any necessary maintenance issues of housing units in a timely fashion. Assist with the coordination of the on-site food pantry, including inventory management, food ordering, and distribution to residents. Maintain accurate records and submit required reports to Helping Harvest in compliance with their guidelines and reporting schedules. Ensure food safety standards are upheld and that pantry services are delivered with dignity and respect to all participants. Attend recommended training and workshops. Shows an attitude of cooperation and support as a member of the team. Respects the confidentiality of records pertaining to residents. Fulfills Department of Human Services annual training. Performs other job-related duties as assigned.
QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Minimum age 18 years Bachelor's degree in social services or comparable field preferred with a minimum of one-year direct service experience in crisis intervention, case management, shelter/transitional emergency/transitional housing, and/or substance abuse.

Degree can be substituted for equivalent experience. Excellent communication and organizational skills with basic computer knowledge Ability to work independently, accurately and efficiently, with an eye for detail Excellent phone manners and customer service skills Self-motivated and able to work independently. Valid Pa Driver's License and good driving history. Ability to report to work in inclement weather. Ability to make logical decisions that are in the best interest of residents and the program. Ability to multi-task and work in a fast-paced environment. Work effectively with individuals who have diverse styles, abilities, motivations, and backgrounds. Adapt to change productively and handle other tasks as assigned. Arrive to work promptly and regularly. Ability to make logical decisions that are in the best interest of residents and the program.
