Job Description
Community Quest is a non-profit organization, operating 3 group homes and 2 supervised apartments in Southern NJ. In the role of a Team Lead, you would be working with developmentally disabled individuals in a group home setting. Some of the responsibilities of the Team Leader include: Assisting consumers with personal hygiene such as bathing, feeding, toileting Being a mentor for Residential Aides and ensuring team members are providing quality care. Completing detailed daily documentation and participating in decision making processes. ISP Coordinator for assigned Group Home;•develops and implements ISP Ensure individual's health needs are being met, schedule, follow up and accompany to appointments Identifies and assists in the provision of pertinent documentation as per Agency and Division requirements including staff development and training of Residential Aides Attends annual licensing inspections and coordinates the development and implementation of plan of corrections Submits individual support case notes weekly BSP Coordinator for assigned individuals;•implements BSP and monitor progress Provides direct supervision to Residential Aides Heavy lifting, transporting of service recipients and moving of equipment, etc. may be required Health and Safety Committee member; ensures compliance with Agency Policies and Procedures and Health and Safety Policies and Procedures Ensure accurate and complete documentation on Individual Service Recording system Volunteer in agency fundraising events and sit on agency committees as needed Work flexible schedule covering shifts as needed to ensure adequate supervision of service recipients in the program Responsible for the accuracy of Medication Administration Record Sheets (MARS) and inventory Assume additional responsibilities in absence of the Residential Manager Reports to specific work locations determined by residential program needs Ensure all content of service recipients main file book is current Responsible for the completion of monthly medication report, fire drills, program safety checks and weekly menus Responsible for the accuracy of Medication Administration Record Sheets (MARS) and inventory Audit Medication Administration Record Sheets (MARS) Full Time•40
