Pathways to
Housing PA, Housing First Services Position Summary:
The Team Leader will manage the daily operation and clinical supervision of a non-fidelity Assertive Community Treatment (ACT) team. The Team Leader is responsible for the development and oversight of a program to meet the needs of the team participants who present with an array of challenges connected to being homeless: psychiatric disabilities, substance use disorders, homelessness, untreated and complex medical conditions, lack of adequate support, etc. The Team Leader is responsible for assuring that the staff exercise person-centered and harm reduction approach aligned with the agency's mission and core values. The Team Leader maintains oversight to ensure that client self-identified goals for recovery, rehabilitation and community inclusion are supported by appropriate services and best practices.
Essential Duties and Responsibilities:
Maintain a Team comprised of professional, para-professional and support staff Maintain ACT like core policies, practices and services Ensure compliance with all accreditation, certification and regulatory standards Train, supervise and evaluate the team in conjunction with the Clinical Director Plan and implement all client services, in collaboration with team members Support harm reduction model by providing non-judgmental, non-coercive services and resources to individuals engaging in risky behaviors Provide data for written reports requested by city, state, or federal funding sources Supervise electronic and written record-keeping activities, e.g. databases, on-call logs, incident reports, progress notes and client charts Coordinate services of the team with other inpatient and outpatient community treatment, support and social services. Identify and develop appropriate services for unmet needs Establish operational procedures and monitor client visits and team assignments Collaborate with Pathways to Housing departmental staff to assure optimal apartment placement and maintenance of safe and comfortable living environment for clients Provide direct services to clients approximately 50% of the time Perform other duties as assigned
Essential Education, Experience and Skills:
Patience, creativity, flexibility, compassion, and sensitivity to persons with disabilities and other minority populations Master's in Social Work preferred or Master's in another clinical mental health discipline, such as: psychology, counseling, clinical social work, psychiatric nursing, or marriage and family therapy accepted; LCSW or actively working towards LCSW preferred Five years mental/behavioral health direct care experience, including three years supervisory experience required Responsible, organized and efficient Excellent interpersonal, communication and writing skills, as well as computer literacy Previous experience in case management/counseling of people with psychiatric disabilities, histories of substance abuse and/or trauma Ability to work with diverse populations including clients and staff in critical and emergency situations Sensitive to cultural, economic, gender, and sexual orientation difference Valid driver's license and clean driving record
Supervisory Responsibilities:
All employees in assigned team.
Physical Demands and Work Environment:
Community environment. Considerable walking and standing may be required.
