care Social work care coordination Patient education Business Associate's degree Case service coordination 2 years Communication skills Healthcare data reporting Full Job Description Carries out the day-to-day activities of a patient referral and care coordination service for the Ryan White HIV Program, including facilitating access to specialty and dental care, providing education and health literacy support to clients, and maintaining program data systems. This position provides direct navigation, support, and coordination services to individuals living with HIV, working collaboratively with the University of Utah Infectious Disease Clinic, interdisciplinary care teams, and community partners. The role supports clients in accessing and maintaining engagement in care through referral coordination, program education, and assistance with service utilization and billing processes. The position also supports program operations through data tracking, reporting, and monitoring activities to ensure effective service delivery and compliance with program requirements. Responsibilities Provide direct navigation, mentoring, and support to individuals living with HIV to facilitate linkage to and retention in care. Maintain knowledge of available services, eligibility criteria, and community resources relevant to the Ryan White population. Monitor referrals and follow up to ensure successful access to services. Identify barriers to care and assist clients with problem-solving and resource connection. Support client engagement by promoting understanding of care plans and available services. Attend internal and external meetings with program partners and stakeholders. Contribute to program quality improvement and reporting activities as needed. Perform other duties as assigned. Minimum Qualifications Associates degree in Business, Finance, Health Care Administration or a related field, or equivalency (one year of education can be substituted for two years of related work experience); demonstrated human relation and effective communication skills also required. This position is patient-sensitive and must fulfill all associated requirements.
