We take a concierge-style approach to care: every client is paired with a dedicated Patient Coordinator for a seamless experience from consultation through treatment, across 150+ clinic locations nationwide. Behind that experience is our in-house clinical practice-management platform, which our engineering team builds and operates. It covers the full patient lifecycle, lead, lab, encounter, orders, and ongoing care including provider assignment, lab ordering and results, testosterone/HRT prescribing and regimen management, appointments, payments, patient forms, and the audit/compliance surfaces required to handle protected health information (PHI) responsibly. About the Role We're hiring a senior engineer to both build product in our TypeScript codebase and own AWS infrastructure for our beta and production environments.

This is a high-agency role:

you'll design domain models, ship APIs and UI, harden deployments, and help keep our healthcare platform reliable and secure. You'll also lead the ongoing development of Renew Vitality's proprietary EMR, driving new features and new projects as we expand into new business lines. What You'll Do Lead future development of Renew Vitality's proprietary EMR, driving new features, new projects, and new business lines Deliver full-stack features in a TypeScript pnpm monorepo (Express + tRPC backend, React/Vite frontend) Build domain-heavy backend logic, lab ordering, e-prescribing, appointments, billing with a strong preference for pure, testable functions Own day-to-day operation of AWS (ECS, ECR, S3, IAM, networking, CloudWatch) across beta and production Integrate with third-party clinical and payment systems (e-prescribing, lab vendors, payment processors) and keep production healthy Improve reliability: deploys, rollbacks, health checks, job processors, and vendor failure handling Write unit and integration tests, and document non-obvious product and architecture decisions Requirements 5+ years of professional software engineering experience with senior-level ownership of complex systems Strong TypeScript skills, both backend and frontend Experience with Node.js/Express (or similar), PostgreSQL, and modern React Solid AWS experience: containers (ECS/ECR), S3, IAM, networking, and monitoring comfortable owning infrastructure day-to-day Comfort with CI/CD (GitHub Actions), Docker, and production debugging Clear written communication; able to work from specs/ADRs and leave the codebase better than you found it Nice to Have tRPC, Prisma, Zod, or the TanStack ecosystem (Router/Form/Query) Event sourcing, CQRS, or functional/onion architecture experience Healthcare, HIPAA/PHI, payments, or e-prescribing experience AWS CDK (or Terraform/CloudFormation), SES/SNS/SQS, CloudWatch Tech Stack TypeScript
