Full Job Description Therapist, Montgomery County ACT Team Salary :
$75,000
  • $85,000 Pathways to Housing DC is an innovative agency based in the heart of Washington, D.
C with services in Montgomery County Maryland. We believe that housing is a basic human right, and that without housing people are unable to address their mental health disorders, addictions, and other life stressors. We provide each person entering our program their own apartment, without requiring treatment compliance or sobriety as a prerequisite. We then provide each person with intensive support services to help them stay housed and begin their journey to recovery. The Assertive Community Treatment (ACT) team provides flexible, community-based services that are designed to promote the stability, recovery and community reintegration of formerly homeless individuals who have a mental illness and co-occurring substance use disorders. The ACT Therapist is responsible for providing clinical, community-based mental health services to people experiencing/formerly experiencing homelessness in Montgomery County.
What you will do:
Complete clinical assessments on new and ongoing clients Provide therapeutic services to consumers in the community and in their home Provide community-based outreach services to homeless consumers Provide individual, family and group therapy to targeted clients within the program. Maintain clinical documentation of all consumer visits in a timely and high-quality manner Participate in the coordination of the consumer admission & assessment processes. Developing, writing, implementing, and revising consumer treatment plans, crisis plans, and discharge plans. Assist in the teaching of daily living skills; this includes teaching and assistance in services such as grocery shopping, maintaining housing, utilizing public transportation, maintaining self-care, exploring employment opportunities, etc. Completes monthly outcome reports on case plans, Provides other related duties as required.
Qualifications:
Maryland licensed LMSW, LGPC, LCSW-C or LCPC Experience in case management or counseling of people with psychiatric disabilities, histories of substance abuse and/or trauma. Previous experience with ACT is highly desired. Demonstrated knowledge of mental health and substance abuse clinical skills Patience, creativity, flexibility, compassion, and passion for working with persons with complex needs Career experience in social services, supported housing, or with persons with disabilities; Excellent organizational, communication, writing, and computer skills; Bilingual in English/Spanish Strongly Preferred (Bonus Offered) Driver's License required Position Highlights Community-based clinical role within a nationally recognized Housing First ACT program Opportunity to make a direct impact supporting individuals with complex mental health and housing needs Collaborative, mission-driven team focused on recovery, stability, and community reintegration
