The Transition Coordinator coordinates day-to-day operations of transition to adulthood (employment, volunteerism, service, independent living, leadership, advocacy, and post-secondary education) programs and services. This position works collaboratively with the executive coordinator for transition and provides guidance to the transition specialist in developing, implementing new programs services and activities; maintaining existing programs, services and activities; monitoring person-centered plan (plan of care) development, service coordination/case management, and internships; and overseeing direct training and technical assistance to youth and young adults with disabilities, family members and those that serve them. Primary Duties and Responsibilities Researches, reviews and implements transition to adulthood best practice and research-based recommendations into transition to adulthood training and technical assistance activities.
Designs, provides oversight and oversees quality monitoring of transition to adulthood best practice and research-based training to youth and young adults with disabilities, families, providers and the business community/employers.
Coordinates and provides staff support and guidance in the dissemination of information regarding best practices at state, regional, or national trainings and conferences.
Cultivates partnerships with local, state, and national organizations/agencies, as well as University staff, faculty, and departments to promote transition services.
Coordinates compliance with internal program policies and regulations for all transition services.
Works and provides guidance to transition staff to develop schedules, program structure, and training curricula.
Documents and tracks all transition activities including data collection and quality monitoring for use in preparing reports for IDS and funding agencies.
Provides guidance for reporting and monitors implementation of transition services including usage of intake forms, needs assessment tools and individual support plans
Monitors staff provision of advocacy support and self-sufficiency development to individuals with disabilities
Performs other duties as assigned. Minimum Qualifications Bachelor's degree from an accredited college or university in disability studies, social work, healthcare, special education, psychology, child and family sciences, counseling or related field. Three to five years of experience or training beyond a bachelor's degree in working with persons with disabilities providing transition training, technical assistance and/or direct services. Ability to analyze and prepare documents, reports, and correspondence,

Skill in being detail-orientated and timely with all required notes, files and documentation,
Ability to establish goals and objectives, and
Ability to be resourceful and determined to provide a positive experience for others.
Preferred Qualifications
Master's degree from an accredited college or university in disability studies, social work, healthcare, special education, psychology, child and family sciences, counseling or related field. Social work licensure or case management certification. About The University of Southern Mississippi
