THE PROGRAM
: Credible Messenger Program The Credible Messenger program aims to assists youth and families with identified needs related to behavioral health, substance use, medical care, housing, education, employment, legal matters, transportation, benefits, family support, and other activities of daily living, while completing administrative, programmatic, triage, and documentation responsibilities in accordance with program and contract expectations. Join a collaborative team where every day is an opportunity to learn, grow, and change lives! Duties & Responsibilities Coordinate intake, engagement, and ongoing services for youth referred by
CSSD/JBCSSD
in accordance with program contract requirements and the Individual Service Plan. Serve as a liaison between CHR, CSSD/JBCSSD, clients, families, natural supports, and community providers to support communication, service access, treatment planning, and discharge planning. Link clients and families to community supports and resources, including behavioral health, substance use, medical care, housing, benefits, legal services, education, employment, and other identified needs. Assist clients with appointment scheduling, transportation coordination, and other practical supports that promote engagement and reduce barriers to care. Maintain accurate, timely documentation of services, contacts, referrals, and communications in compliance with agency, program, and contract standards. Participate in supervision, team meetings, trainings, and other meetings as assigned. Perform other duties as required.
QUALIFICATIONS
Education:
High School Diploma or equivalency program (GED)
Experience:
Credible Messengers will come from and live in the communities of the youth to be served, shall have lived similar, relevant experiences of the youth, and have transformed their own lives.
Licensure/Certification/Registration:
Valid driver's license required.
Training:
Will attend all required program trainings for CHR and CSSD Why Join CHR?

COMPENSATION
•: Starting at $19.2850 min., and up commensurate upon experience. Actual rates are determined at the time of offer and are based off of relevant experience, as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives.
