Job Description

Position Summary:
The Community Engagement Specialist provides supportive, person-centered assistance to individuals enrolled in waiver programs, helping them access community resources, activities, and social opportunities. This role focuses on promoting independence, skill-building, and community inclusion while ensuring compliance with program and safety requirements.
Key Responsibilities:
Community Support & Engagement Accompany participants to community outings, appointments, educational programs, and recreational activities. Facilitate social interaction, skill development, and participation in community events. Support individuals in accessing resources (libraries, community centers, healthcare, employment services). Encourage independence and self-advocacy while ensuring participant safety. Waiver Program Compliance Document all services provided in accordance with waiver program requirements. Complete progress notes, reports, and activity logs accurately and timely. Follow individualized service plans (ISPs) and guidance from case managers or program supervisors. Maintain confidentiality and adhere to HIPAA and agency policies. Communication & Collaboration Coordinate with family members, caregivers, case managers, and other support staff. Provide feedback to supervisors on participant progress, barriers, and successes. Advocate for participants' needs and preferences within the community and organizational programs. Administrative Duties Track mileage, activities, and community engagement hours as required by funding sources. Maintain organized records of outings, communications, and participant progress. Assist in planning and organizing group activities or special community events.
Qualifications:
High school diploma or equivalent required; some college in social services, psychology, or related field preferred. Experience working with individuals with disabilities, mental health needs, or behavioral health conditions preferred. Valid driver's license and reliable transportation. Excellent interpersonal, communication, and problem-solving skills. Ability to follow individualized service plans and maintain professional boundaries. Knowledge of local community resources and services is a plus.

Job Types:
Full-time, Part-time Pay:
$15.00 - $18.00 per hour
