ABOUT FIVE RIVERS HEALTH CENTERS
Five Rivers Health Centers is an expanding Federally Qualified Health Center (FQHC) providing a medical home for low-income patients in Dayton and Xenia, Ohio. We provide quality medical, dental, vision, and behavioral healthcare services to over 27,000 patients with more than 90,000 visits annually at six convenient locations. Position Overview The Transportation & Delivery Personnel must abide by various regulatory compliance rules and must always treat patients with dignity and respect. The driver will deliver prescriptions and transport patients to/from appointments. General Responsibilities Transport medications between the pharmacy, patient residences, and clinic locations using efficient routes to ensure timely and accurate deliveries. Maintain the proper storage, handling, and security of medications in accordance with policies, procedures, and regulatory requirements. Operate company vehicles safely and responsibly, ensuring adequate fuel levels, and reporting maintenance or service issues. Verify and update patient insurance information in the electronic health record. Obtain required documentation and maintain records of deliveries and patient interactions. Ensure deliveries are completed accurately, confidentially, and in compliance with HIPAA and Five Rivers Health Centers standards. Escalate patient questions, concerns, or issues to supervisor and appropriate pharmacy staff. Qualifications Two (2) years of transportation experience required. A valid Ohio driver's license is required for this position. Employment in a driving role is contingent upon the applicant's ability to meet the insurance carrier's requirements for automotive insurance coverage. High School diploma or equivalent preferred.
