Ability to perform evaluations, develop plans of care, and provide home health physical therapy in compliance with state laws and agency policies. Experience documenting in electronic medical records. HCHB (HomeCare HomeBase) experience preferred. Home health experience preferred.

Responsibilities include patient evaluation, development and implementation of PT plan of care, progress documentation per agency policy, care coordination with physicians and interdisciplinary team, patient and caregiver education, and working with diverse home environments and patient populations.
