Join a traveling Physical Therapist role in Fort Morgan, CO (Home Health) where you will help patients recover from injuries and manage chronic conditions while providing skilled care in patients' homes. (Minimum education: Master's required per client.) Active Colorado Physical Therapist license required to start the assignment. Home health experience with OASIS documentation required. Demonstrated experience performing comprehensive patient evaluations, developing skilled plans of care, and documenting skilled necessity. Proven skills delivering home-based interventions including therapeutic exercise, gait and balance training, transfer and stair training, and caregiver training. Experience evaluating home safety and fall risk and implementing environmental modifications. Ability to coordinate care with interdisciplinary teams and communicate with physicians and referral sources.

Certifications:

BLS from AHA or ARC may be required.

Preferred:

Experience with HCHB electronic documentation system.
