Active Colorado Physical Therapy license is required to start the assignment. Home Health OASIS clinician experience is required. Experience providing in-home skilled physical therapy, including evaluation, plan of care development, therapeutic exercise, gait and balance training, transfer training, and fall prevention. Experience documenting OASIS components, visit notes, reassessments, and discharge summaries. BLS certification from AHA or ARC may be required. Ability to travel within a 50-mile radius of Fort Morgan, CO for patient visits.

Preferred:

HCHB (Home Care Home Base) electronic medical record experience.
