Travel Looking for a change of scenery and a chance to help others. Join as a traveling Physical Therapist providing home health care and help patients recover from injuries and manage chronic conditions while experiencing life in a new location. Apply now and a recruiter will reach out with more details. Minimum Requirements Master's (MPT) or Doctor of Physical Therapy (DPT) degree from an accredited program. Active Colorado Physical Therapy license is required to start the assignment. Home Health OASIS clinician experience is required. Experience providing in-home skilled physical therapy, including evaluation, plan of care development, therapeutic exercise, gait and balance training, transfer training, and fall prevention. Experience documenting OASIS components, visit notes, reassessments, and discharge summaries. BLS certification from AHA or ARC may be required. Ability to travel within a 50-mile radius of Fort Morgan, CO for patient visits.
Preferred:
HCHB (Home Care Home Base) electronic medical record experience.

up to a 50-mile radius of Fort Morgan, CO. Mileage reimbursed between visits from first patient to last patient.

Professional certification required:
MPT or DPT.
Minimum education:
Master's degree.
