Qualifications:
Current state licensure as a Physical Therapist (PT) in good standing Clinical competency in evaluating, establishing, and revising therapy plans for hospital-based outpatients Strong documentation, goal-setting, and charge entry skills appropriate to regulatory standards Professional commitment to ongoing education and adherence to ethical conduct Excellent communication and team collaboration abilities
Key Responsibilities:
Perform comprehensive patient evaluations, develop and implement individualized therapy plans in line with physician direction and patient goals Delegate and coordinate discipline-specific therapy services Design functional treatment programs aimed at effective, community-based patient recovery Maintain timely, accurate, and compliant medical record documentation Promote safety in all aspects of patient and clinic practice, identifying hazards and following established protocols Serve as an educator for patients, families, and colleagues, and actively participate in student supervision and professional development initiatives
