Qualifications:
Valid and active Physical Therapy license (in good standing) for the state of Connecticut Experience in hospital or outpatient therapy preferred Familiarity with safe patient handling, infection control, and workplace safety protocols Commitment to professional codes of conduct, ethical patient care, and regulatory compliance Exceptional communication and organizational skills
Key Responsibilities:
Conduct comprehensive patient evaluations, develop and adjust individualized treatment plans based on patient goals and physician direction Delegate and direct discipline-specific therapy services, ensuring high-quality standards Facilitate smooth transitions for patients back to their community activities through functional treatment programming Foster patient and employee safety, utilizing provided equipment and adhering to protocols for handling, infection control, and environment safety Accurately document all medical records in a timely manner following federal, state, and clinic guidelines Mentor students, educate patients and families, and engage in ongoing professional development Participate in continuous quality improvement efforts, offering constructive feedback and implementing action plans
