Schedule Day shifts only No weekend requirement Holidays off Key Responsibilities Provide physical therapy evaluations and treatments in a home health setting, covering most of Tehama County. Manage a daily caseload averaging 5 visits, primarily with orthopedic/joint replacement and complex geriatric patients. Perform SOC (Start of Care) visits, especially for ortho patients without concurrent nursing services. Use the HomeCare HomeBase (HCHB) EMR system on a provided tablet for all documentation. Collaborate with Intake RNs, managers, and nurse aides as part of the interdisciplinary team. Travel approximately 40 miles per day within your service area (expected to decrease with increased staffing). Qualifications Current BLS certification. PT license in California (or eligibility). Experience in home health strongly preferred (open to less than two years of overall experience if proven in home health).
