• Nursing Home Responsibilities Conduct comprehensive physical therapy evaluations for residents in a long-term care setting to determine functional status, rehab needs, and goals.

Develop and implement individualized treatment plans focusing on mobility, strength, balance, pain management, and activities of daily living. Deliver direct PT interventions, including therapeutic exercises, gait training, manual therapy, neuromuscular re-education, and modalities as appropriate. Monitor patient progress, document outcomes, adjust treatment plans as needed, and communicate results to the interdisciplinary care team. Ensure safety, infection control, and regulatory compliance in all therapy activities; uphold patient privacy and professional standards. Collaborate with nursing and therapy staff to support discharge planning and transitions of care for residents.
