5x8 Days Start date: 05/11/2026 Work setting: Skilled Nursing Facility (SNF)/Long-Term Care travel assignment Responsibilities Conduct initial and ongoing assessments of residents' mobility, balance, strength, gait, and functional limitations; develop individualized treatment plans to restore or improve function. Deliver direct therapy interventions, including therapeutic exercises, gait training, balance retraining, and the use of modalities as ordered by the physician. Monitor resident progress and adjust care plans to ensure treatment goals are met; participate in discharge planning as appropriate. Educate residents, families, and caregivers on home exercise programs and safety strategies to promote independence. Supervise and guide Physical Therapist Assistants (PTAs) and therapy aides in accordance with licensure requirements; accurately document sessions, progress notes, and billing information; ensure compliance with state, federal, and facility standards.
