Qualifications & Desired Experience:
Current RN license, eligible to practice in Michigan Prior experience in rehabilitation or long-term care preferred Strong assessment, documentation, and patient education skills Ability to collaborate effectively with interdisciplinary teams Resourceful, detail-oriented, and adaptable in high-acuity settings
Responsibilities:
Complete initial nursing assessments within 8 hours of admission for all patients Collaborate with families and care teams to develop individualized plans of care Participate actively in patient rehabilitation programs and team conferences Provide clear, accurate, and timely documentation in accordance with policy Ensure patient and staff safety by adhering to safety policies and protocols Offer supervision and support to ancillary nursing personnel Communicate critical changes in patient status to providers for seamless care transitions Proactively engage in continuing education and professional growth activities
