Schedule/Hours:

10-hour day shifts, 8:00 AM - 6:00 PM Shift options: Thursday-Sunday, or Saturday-Tuesday Guaranteed 40 hours per week (except during certain holidays or orientation)

Qualifications:

Active WI RN license BLS certification (American Heart Association) Valid driver's license 1-2 years of recent home health and hospice experience Demonstrated proficiency with OASIS Recent experience with Epic charting (within 12 months) Strong time management skills; resilient, flexible, and social Physically able to manage stairs, uneven terrain, and drive between visits Able to work in homes with potential pet or smoke exposure Covid-19 vaccination required (medical/religious exemptions considered)

Key Responsibilities:

Provide skilled nursing assessments and interventions in a home setting for adults managing chronic illnesses including CHF, COPD, diabetes, cancer, CVA, wound care, and IV administration Conduct admissions, death visits, or symptom management for hospice patients Deliver timely, accurate OASIS documentation Navigate varied home environments and rapidly changing schedules Maintain professionalism in both routine and challenging situations
