As a floating case manager, you will step into different care teams, assist with growing census, and provide stability during transitions. In this role, you will provide skilled nursing care in the home, complete assessments, develop care plans, and coordinate services to ensure patients receive the highest level of care. You will also assist different territories as needed, helping maintain continuity of care during growth or staffing changes. Floating RN Case Manager Responsibilities Provide skilled nursing visits and comprehensive patient assessments. Develop, implement, and update individualized plans of care. Educate patients and caregivers on medications, safety, and disease management. Support multiple territories based on patient and staffing needs. Assist with admissions, routine visits, and case management coverage. Collaborate with therapists, social workers, and physicians. Complete accurate documentation including OASIS when required. Participate in rotating on-call coverage. RN Case Manager Qualifications Active Minnesota Registered Nurse license. Valid driver's license and reliable vehicle. Strong clinical judgment and care coordination skills. Ability to work independently in a field-based setting. Home health experience preferred but not required.
