TRAVEL/LOCAL CONTRACT - LTC Licensed Practical Nurse (LPN)

This contract offers a 13‑week assignment as a Licensed Practical Nurse in a long‑term care setting near Warren, Minnesota. The role focuses on delivering direct patient care, collaborating with interdisciplinary teams, and maintaining accurate documentation in EMR systems.

Key Responsibilities:

Provide direct patient care within LPN scope, including medication administration and routine assessments Collaborate with interdisciplinary team members to develop and implement individualized care plans Maintain accurate and detailed documentation of patient conditions, treatments, and interventions within EMR systems Educate patients and their families on health management, medications, and care procedures Ensure compliance with all facility policies, safety protocols, and state regulations during shift operations

Qualifications:

Completion of an approved Practical Nurse training program from an accredited institution Valid Licensed Practical Nurse (LPN) license in Minnesota CPR certification At least 1 year of recent experience working in a Long Term Care setting Familiarity with electronic medical records (EMR) systems Ability to work evening and night shifts Excellent communication and interpersonal skills Ability to stand for extended periods Reliable transportation Strong attention to detail

Preferred Skills:

Infection Control certification (preferred) Experience with evening or night shifts (preferred)
