Universal Worker /Direct Care Worker

Duties Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor residents' health status by observing vital signs, patient behavior, and overall condition; report changes promptly Support medication administration following established protocols and HIPAA regulations to ensure privacy and safety Prepare nutritious meals and assist with meal service, ensuring dietary needs and preferences are met Maintain cleanliness of resident rooms and common areas through regular cleaning and sanitation practices Document care activities accurately in resident care plans and progress notes; participate in care plan updates Provide emotional support and companionship to residents, especially those with dementia or memory impairments such as Alzheimer's disease or autism spectrum disorder Follow safety procedures including CPR, First Aid, and emergency response protocols to ensure resident safety at all times Collaborate with nursing staff and other healthcare professionals to deliver comprehensive care tailored to each resident's needs Support residents' participation in social activities and community engagement efforts to promote mental stimulation and quality of life Qualifications Valid CNA certification or equivalent licensure a plus but not required; prior experience in assisted living, nursing home, or home & community care preferred Demonstrated knowledge of patient observation techniques, vital signs monitoring, medication administration, and HIPAA compliance Experience working with individuals with developmental disabilities, dementia (including Alzheimer's), autism spectrum disorder or in hospice care is highly desirable Physically capable of assisting with resident transfers and mobility support Strong communication skills for effective interaction with residents, families, and team members Compassionate attitude with a genuine desire to improve residents' quality of life through direct support and caregiving activities Knowledge of infection control practices including cleaning protocols and safety procedures such as CPR and First Aid certification is preferred Flexibility to work various shifts including weekends or holidays as needed; reliable attendance is essential Join us in creating a caring environment where every resident feels valued and supported.
