Job Description
Possibilities Service Provider Summary of Job:
Responsible for providing individual support and assistance to clients within our community. Responsible for the supervision, training, and personal care needs of clients while attending Goodwill Possibilities.Supervision From:
Senior Regional Manager-Marinette Supervision To:
None Work Performed:
Essential functions: 1. Provides assistance in various settings. These services may include but are not limited to: daily living tasks, health maintenance, housekeeping, recreational activities, etc. 2. Maintain accurate records of hours and services provided and document daily. 3. Provide local transportation and occasionally long distance transportation for consumers served, as requested. 4. Development of new and existing curriculums for Possibilities. 5. Maintain confidentiality, personal privacy, recipient rights, HIPPAA, and other similar practices.General duties:
1. Responsible for implementing safety procedures and for safety of individuals and the reporting of any unsafe conditions observed. 2. Responsible for maintaining a clean work area. 3. Develop community relationships and suggest activities that strengthen community integration for persons that Goodwill serves. 4. Other duties as assigned by the Supervisor.Knowledge, Abilities and Skills:
1. Working knowledge of social concerns pertaining to service. 2. Working knowledge of the principles of human growth and behavior. 3. Ability to develop and carry out service plans for those served. 4. Ability to establish and maintain healthy relationships with customer, families, affiliated agencies, etc. 5. Ability to maintain CARF standards of performance. 6. Demonstrate technology/computer skills to enter accurate, timely documentation of services. 7. Able to provide transportation to customers on a regular basis.Qualifications:
1. High school diploma or GED required. 2. 1 or more years of work experience or related education.Similar remote jobs
