We currently have an opportunity for a Home Care Aide Respite Supervisor in Forest Park, IL. The role of the Respite Supervisor is to provide the day-to-day coordination, scheduling and supervision of the City of Chicago Respite Program. In addition, the Respite Supervisor is responsible for intake of cases from referral sources and placement of appropriate personnel for each case. Respite Supervisor is responsible for inputting all referrals and maintaining the City of Chicago AgingIS referral database.
Key Functions include:
1. Accept intake of cases for the respite program from referral sources. 2. Schedule appropriate and qualified personnel for client assignments. 1. Interact with the City of Chicago for the following: Attend monthly delegate meetings. Monthly reporting of referrals received and input in the AgingIS referral database. Obtain/verify all facility stay invoices from vendors and forward to the accounting department by the 28th of each month for the prior month respite stays. Participate in monthly outreach activates to include but not limited to public presentations, brochures, flyers or other methods to reach a broad cross section of family/informal caregivers throughout the city. Coordinate satisfaction survey activities. Surveys are provided to each client once their services have ended. 4. Accept grievances and complaints from clients and personnel and complete the appropriate and required forms and forward to the applicable parties. 5. Schedule and update all cases on a daily basis in the agency computer system. 6. Interact with clients and client families to ensure quality care and client satisfaction, in accordance with HIPAA requirements. 7. Verify smooth transition from office hours to on-call staff personnel through the preparation of on-call reports and verbal communications.
Qualifications:

Educational:
College degree preferred or equivalent work experience 2.
Certification:
Will be required to complete the TCARE Care Manager Training. 3.
Travel:
Travel is required. The employee must have a valid driver's license issued by the state in which they work and a satisfactory driving record. 4.
Work Experience:
a. A minimum of two years' work experience. b. Ability to use computerized systems. c. Satisfactory verbal and written communication skills. d. Ability to perform supervisory activities. e.
