Position Summary The Something More Intake Specialist supports individuals experiencing housing instability, chronic health conditions, justice involvement, or other complex life circumstances by completing high-quality intake assessments and connecting members to appropriate housing and ECM services. This role ensures that every member receives a respectful, trauma‑informed, culturally responsive introduction to services. The Intake Specialist gathers eligibility information, verifies documentation, completes initial assessments, and coordinates warm handoffs to Housing Navigators, Care Managers, and community partners. This is an entry-level position ideal for candidates who are passionate about equity, community support, and person-centered care. Responsibilities Conduct bilingual (Spanish) intake appointments via phone, in person, or in the field. Provide trauma-informed, culturally respectful communication at all times. Build rapport with members while maintaining professional boundaries. Complete standardized intake forms, housing assessments, and ECM assessments in alignment with CalAIM requirements. Identify immediate needs related to housing, safety, health, transportation, food access, and behavioral health. Screen for Housing/ECM eligibility and document acuity indicators. Collect and verify required documents, including IDs, income verification, medical documentation, and housing-related paperwork. Enter all intake data accurately into the electronic health record or case management system. Coordinate warm handoffs to Housing Navigators for ongoing support Communicate member needs, barriers, and motivators to the assigned Care Manager. Maintain accurate, timely, and complete documentation for all intakes and interactions. Document all outreach attempts in compliance with organizational and CalAIM standards. Follow HIPAA, HITECH, and organizational privacy standards. Qualifications Organization and attention to detail Problem-solving and resourcefulness Lived experience or strong ties to the communities we serve Bilingual Spanish/English fluency is a requirement. High school diploma or equivalent. Strong communication skills, including the ability to explain processes clearly and compassionately. Ability to conduct fieldwork, including visits to shelters, libraries, hospitals, and community sites. Ability to sit, stand, or walk for extended periods. Comfort working with individuals experiencing homelessness, chronic illness, behavioral health needs, or justice involvement. Ability to maintain professionalism, confidentiality, and cultural humility. Basic computer skills and ability to learn case management systems. Valid driver's license and reliable transportation for field-based work Experience working with diverse populations, including individuals with lived experience of homelessness, trauma, or reentry.
