Home Health Intake Specialist Full-Time | Monday-Friday | Leesburg, FL A growing Medicare-certified home health agency is seeking an experienced, organized, and dependable Intake Specialist to join our team. The Intake Specialist is responsible for managing new patient referrals from receipt through acceptance and coordination of Start of Care. Responsibilities Receive and process incoming home health referrals Review referrals for completeness and required documentation Verify patient demographics, insurance/Medicare eligibility, diagnoses, physician information, and orders Enter referrals accurately into Axxess Communicate with hospitals, SNFs, physician offices, patients, and referral sources Obtain missing orders, records, and documentation Coordinate accepted referrals with the clinical team for timely Start of Care Track pending referrals and follow up until completed Maintain accurate and organized patient records Assist with physician orders and intake-related documentation Qualifications Home health intake or healthcare experience strongly preferred Axxess experience is a major plus Knowledge of Medicare home health referrals and documentation preferred Proficient with Microsoft Office, including Outlook, Word, and Excel Strong computer and data-entry skills Excellent written and verbal communication skills Strong attention to detail and follow-through Ability to manage multiple referrals and priorities in a fast-paced environment Dependable, professional, and highly organized

Pay:

$24.92 - $30.01 per hour Expected hours: 40.0 per week
