Overview As a Patient Enrollment Specialist , you will serve as the initial point of contact for eligible patients, introducing them to our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs. You will conduct compassionate outbound outreach to educate patients on the benefits of these services, address their questions, and guide them through a seamless enrollment process. This is a full-time, onsite or remote position, ideally suited for engaging communicators with a strong background in phone outreach, customer service, or patient navigation.
Key Responsibilities Outbound Outreach:
Initiate outbound phone calls to pre-qualified, eligible patients referred through our medical practice partnerships.
Patient Education:
Clearly and compassionately explain program benefits (such as 24/7 care team access, personalized care coordination, and remote device tracking) to patients and their family members.
Enrollment & Onboarding:
Guide eligible patients through verbal consent and complete required intake documentation accurately within our platform.
Documentation & Compliance:
Maintain meticulous, real-time records of patient interactions, call outcomes, and enrollment status, ensuring strict adherence to HIPAA and company compliance standards.
Target Achievement:
Consistently meet or exceed established daily outreach volume and weekly enrollment goals.
Qualifications Education:
High School Diploma or equivalent required.
Experience:
1+ years of experience in outbound patient outreach, call center operations, sales, customer service, or healthcare care navigation.
Communication:
Exceptional verbal communication, active listening, and strong phone presence with the ability to build quick rapport with diverse patient populations.
Technical Skills:
Comfortable navigating software applications, CRMs, and EHR systems simultaneously.
Workplace Setup:
Dedicated, quiet home office environment with high-speed, reliable internet.
Preferred:
Active Medical Assistant certification (CMA/RMA), Nursing Assistant certification (CNA), or medical office experience. Prior experience or familiarity with Chronic Care Management (CCM) or Remote Patient Monitoring (RPM) services. Prior patient enrollment experience.

The Care Manager must respect clients and their patients by recognizing their rights, maintaining confidentiality, and working tirelessly to earn and keep their trust. Pay:
$23.00 - $25.00 per hour
