Assistance Home Care is seeking an Intake Specialist to serve as the professional first point of contact for prospective clients, families, and referral partners.

This position manages the intake process from the initial inquiry through documentation, follow-up, and coordination of the family's Discovery Visit.
Responsibilities:
Respond promptly and professionally to new client and referral inquiries. Gather and document complete intake information in WellSky. Route inquiries to the appropriate Assistance Home Care office and team member. Coordinate Discovery Visits, follow-up activities, and internal handoffs. Maintain an organized lead pipeline with clear next steps. Communicate with families using warmth, urgency, empathy, and professionalism.
Requirements:
Two or more years of intake, scheduling, customer service, healthcare administration, home care, or related experience preferred. Excellent telephone, written communication, and follow-through skills. Strong organization and attention to detail. Comfortable managing multiple priorities in a fast-paced office environment. WellSky, CRM, or healthcare software experience is preferred. Valid and active driver's license, auto insurance and registration on a reliable vehicle. Ability to pass an extensive criminal background check and FSCR Ability to lift a minimum of 55 pounds. Ability to pass a drug test with negative panel results What we
