Housing and Self Sufficiency Specialist HOPE Community Services - 3.1 New Rochelle, NY Job Details Part-time 1 day ago Benefits 403(b) Qualifications Spanish Working with immigrant populations Computer literacy English Experience working with individuals experiencing homelessness Full Job Description SUMMARY The Housing & Self-Sufficiency Specialist is responsible for providing outreach, advocacy, support and programming to members of the community and to the clients at HOPE's Homeless Resource Center (HRC). The Specialist assesses individual needs and connects clients to appropriate resources, prepares reports and correspondence, provides follow up services, performs outreach activities, and develops an information and referral network. The Specialist will also engage in on-site visits with HRC clients, perform client assessments, provide supportive and advocacy services and maintain files.
REQUIREMENTS
Applicant should have a high school diploma and 2 years of experience working with populations dealing with homelessness, housing insecurity, and/or food insecurity, or needing mental health services. Must be fluent in Spanish and English and have experience working with immigrant populations. Must be computer literate and should have knowledge of homelessness systems, resources, and mental illness awareness. Knowledge of HMIS (Housing Management Information System) and eLOCCS (Electronic Line of Credit Control System) preferred. DUTIES
  • Assist clients with applications for housing, entitlements and supplemental services such as SNAP, and Medicaid, and assist clients in obtaining any necessary documents, i.e.- SSI award letters, budget letters, etc.
  • Provide follow-up services to ensure that client needs are being met.
  • Maintain current, accurate and confidential records on all clients.
  • Help create programs and build a network of community resources with various NYS and local agencies that align with HOPE's mission.
  • Assess HRC client needs.
  • Record all client interaction in a timely fashion in HMIS and ensure notes are copied to client folder.
  • Update client file folders with notice for required renewals or recertification.
  • Track, maintain and update weekly, monthly and yearly documents.
  • Perform other duties as assigned.
