Bonus Potential Summary/Objective Provide routine testing opportunities, assess patient care needs, education/ disease awareness, and abstract data for grant and program reporting. Essential Job Duties Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Notifies patients of their HIV, HCV and/or STI diagnoses. Provide necessary linkage to care services for substance abuse, mental health services, healthcare for persons living with HIV (PLWH), and comprehensive prevention services, as indicated. Provide individual and relationship-based HIV/HCV and STI counseling and testing. Manage and analyze track, and report data to meet surveillance, evaluation, and reporting objectives of Baptist Hospital and our Grant funders. Prepares and submits descriptive and analytical monthly, quarterly, and annual reports, fact sheets and presentations in a timely manner. Fulfills internal and external data requests, and performs other related work as required. Performs analysis of data, and uses statistics to interpret the significance of data trends. Links the public to resources and information by providing related statistics to the Preventative Screening Dept. community agencies, and the general public. Required Education and Experience High School Diploma or Equivalent Clinical chart review and data abstraction experience preferred 2 years of clinical healthcare experience or 1 year of experience in social work, case management, or health education/promotion Required License/Certifications N/A
