Residential Mental Health & Substance Abuse Treatment Facility About the Role We're looking for a detail-oriented, compassionate Case Manager / Admissions Specialist to join our residential treatment team. This hybrid position blends two critical functions: guiding clients through discharge planning and case management during their stay, and serving as a warm, knowledgeable first point of contact for admissions inquiries and intake calls . The ideal candidate is someone who thrives in a fast-paced clinical environment, communicates with empathy and professionalism, and is equally comfortable coordinating aftercare resources as they are fielding a phone call from a prospective client or their family member in crisis. What You'll Do Case Management & Discharge Planning Facilitate weekly groups on life skills Meet with clients once weekly Develop individualized discharge and aftercare plans in collaboration with clinical staff, clients, and families Coordinate referrals to outpatient providers, sober living, IOP/PHP programs, and community resources Ensure continuity of care by connecting clients with housing, employment, medical, and support services post-discharge Maintain accurate, timely documentation in the client's chart in compliance with facility and regulatory standards Collaborate with therapists, medical staff, and program directors on treatment team meetings Admissions & Intake Answer incoming admissions calls with professionalism, empathy, and urgency Conduct pre-screening to determine appropriate level of care Verify insurance benefits and communicate financial responsibility to prospective clients/families Guide callers through the admissions process from first contact to intake Maintain accurate records of inquiries and admissions in the EMR/CRM system Follow up with leads and referral sources in a timely manner What You Bring Experience working with adult clients in a residential mental health and/or substance abuse treatment setting (required) Prior experience in discharge planning, case management, or aftercare coordination Prior experience with admissions, intake, or phone-based customer service in a behavioral health setting Strong verbal and written communication skills; comfortable on the phone Ability to multitask and prioritize in a dynamic, fast-paced environment Working knowledge of insurance verification and levels of care (detox, residential, PHP, IOP, outpatient) a plus Familiarity with EMR/CRM systems Bachelor's degree in social work, psychology, counseling, or related field preferred (or equivalent experience) Relevant certification/licensure
