Overview:
We are seeking a highly organized and dedicated individual to join our team as a Service Coordinator. As a Service Coordinator, you will play a vital role in program management and administrative tasks to support individuals with developmental disabilities, Seniors and Children with special needs. This is an opportunity to make a meaningful impact in the lives of our participants.
Skills:
  • Ability to oversee multiple projects simultaneously
  • Excellent administrative skills, including record keeping, data analysis, and report writing
  • Experience helpful
  • Strong supervisory skills to provide guidance and support to staff members
  • Exceptional relationship management skills to build rapport with clients, families, and community partners
Duties:
  • Develop and implement programs to meet the needs of individuals
  • Coordinate and supervise service delivery to ensure quality and compliance with regulations
  • Collaborate with internal teams and external partners to ensure seamless service coordination
  • Conduct assessments and develop individualized service plans
  • Provide guidance and support to clients and their families
  • Maintain accurate documentation and records
Qualifications:
  • Experience helpful
  • Strong organizational skills with the ability to prioritize tasks effectively
  • Excellent communication and interpersonal skills for building relationships with clients, families.
  • Ability to work independently as well as part of a team
