We also serve as the Child and Family Connections agency for DuPage, Kane, and Kendall Counties, supporting families eligible for services through the Illinois Early Intervention system. Learn more at www.dayonepact.org. DayOne PACT is growing! We're looking for a Part-Time LMS Service Coordinator to join our Life Management Services team in Lisle, IL. This position primarily supports Special Needs Trust beneficiaries — ensuring trust funds are used to promote quality of life and meet supplemental needs while preserving eligibility for public benefits — along with Advocacy and Standby clients. The ideal candidate is a motivated human services professional with personal finance skills, including budgeting and bill payment, who will follow trust administration guidelines and case management best practices with excellent interpersonal skills and judgment. This is primarily an on-site, in-office position with local travel for client services. This is a part-time, 20-25 hours/week, non-benefited position. What You'll Do
  • Provide service coordination to a caseload of Trust beneficiary, Advocacy, and Standby clients
  • Conduct assessments and routine consultations, develop budgets, manage disbursement requests, and monitor accounts
  • Provide information, referral, and coordination of products and services; visit individuals offsite as needed
  • Coordinate with stakeholders, including service providers, financial advisors, legal professionals, guardians, and vendors
  • Assist in annual accounting and trust tax filing preparations; participate in investment performance reviews
  • Maintain accurate program documentation, including case notes, reports, and fee-for-service billing; maintain billing at 55% of hours worked.
  • Provide back-up service delivery to support Service Coordinator colleagues as needed
  • Participate in supervisions, meetings, and trainings; pursue professional development opportunities as assigned What You Bring
  • Bachelor's degree (required) in Social Work or a related Health & Human Services field (preferred)
  • Experience working with individuals with disabilities and/or case management
  • Strong interpersonal skills and a professional, empathetic approach
  • Personal finance skills, including budgeting and bill payment
  • Excellent organization, attention to detail, and time management
  • Strong computer skills, including Microsoft Office and database systems
  • A valid driver's license and willingness to travel locally (within Cook & Collar Counties) What We Offer
