Job Description

A Self-Directed (SD) Services Coordinator, under general supervision, develops and implements policies and procedures for self-directed services in accordance with the Medicaid Provider Manual and the Michigan Department of Health and Human Services (MDHHS) Technical Guidance. This role will ensure that all new and active self-directed arrangements comply with all applicable state and federal labor laws and policies. This role is responsible for concise collaboration with and training of HealthWest case managers, Fiscal Management Service (FMS) providers, consumers/families, direct hire employee/contract providers, and others (as appropriate) to be fully aware of, and in compliance with, all state and federal requirements related to self-directed services. The SD Services Coordinator will assist in development and coordination of quality assurance processes for the on-going evaluation of self-directed services. As medically necessary, the SD Services Coordinator will act as a Supports Broker, working directly with individuals in service to develop the skills necessary to self-direct their services. 1. Possess a bachelor's degree from an accredited college or university with a major in a human services field, such as psychology, social work, or human services, OR be eligible to graduate within 90 days of application and able to submit an official transcript upon appointment to position. 2. Have one (1) year of experience in providing supports and/or services to persons with mental illness or developmental disabilities; please note, internships count as work experience if they are documented in the Work Experience section of the employment application. 3. Must have reliable transportation that may be used to carry out job duties of this classification. Preferred but not Required 1. Bilingual candidates highly desired. 2. Lived experiences with mental illness/developmental disabilities/substance use disorders valued. 3. Individuals in recovery and individuals with experience in Armed Services valued.

PHYSICAL ACTIVITIES
An employee in this class performs generally sedentary work requiring the occasional lifting of objects weighing thirty-five (35) pounds or less.
ENVIRONMENTAL CONDITIONS
An employee in this class generally works remotely or in the offices of HealthWest, although travel to other County locations and out of the County travel may be required.
