Job Description

Overview Our Lady of Consolation is a proud member of Catholic Health and a recognized leader in providing exceptional quality driven care. We offer Long Term, Post-Acute and Skilled Nursing services. Our Lady of Consolation is located on the beautiful Great South Bay and is comprised of a 345-bed nursing and rehabilitative care center. Our nurses, physicians and support staff are devoted to providing patient driven health care in an environment of compassion to our patients, their families and our community. The Social Work Clerical Assistant provides administrative and clerical support to the Social Services Department in a nursing home or short-term rehab setting. This position assists with resident documentation, filing, scheduling, communication, and coordination of services to help ensure residents receive quality care and support.
Job Details Key Responsibilities:
Provide clerical and administrative support to the Social Services team. Prepare, maintain, and organize resident records, files, and social service documentation. Assist with admission, transfer, and discharge paperwork. Schedule appointments, meetings, care conferences, and resident/family meetings. Answer telephone calls, take messages, and respond to routine inquiries. Assist residents and families by directing them to appropriate staff and resources. Prepare correspondence, reports, forms, and other departmental documents. Track and maintain social service referrals and follow-up activities. Order and maintain office supplies for the department. Maintain confidential resident information in compliance with HIPAA and facility policies. Support compliance with federal, state, and facility regulations. Perform other duties as assigned by the Director of Social Services.
Requirements:
High school diploma or equivalent required. Previous clerical, administrative, healthcare, or long-term care experience preferred. Proficiency in Microsoft Office (Word, Excel, Outlook) and electronic medical record systems. Strong organizational, communication, and customer service skills. Ability to maintain confidentiality and handle sensitive information. Ability to work effectively with elderly residents, families, and interdisciplinary staff. Posted Salary Range USD $23.05 - USD $36.00 /Hr. The pay for this position ranges from $23.05-$36.00 per hour. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
