About the Service Coordinator Role As a Service Coordinator within our Long-term Support Services team, you'll play a vital role in supporting participants who are Medicaid eligible under Community Health Choices waiver through providing ongoing service coordination services to eligible participants including ongoing monitoring to assure individual health, safety, and wellness; linkage to community and available resources; communicating information to participants; and developing and maintaining service plans outlining participant strengths, needs, and services . Starting Rate:
$22.48/hour, per the current union agreement Service Coordinator Core Responsibilities Engage in ongoing assessment of participants include completing comprehensive assessments. Ensure that participants are referred to needed and/or approved services. Develop and maintain plans that outline participant strengths as needs. Provide ongoing monitoring activities to ensure adherence to plans, and satisfaction with services, and determine the ongoing safety and well-being of participants served. Communicate priorities and information to participants and their identified team members. Service coordinators monitor and ensure that assigned participants maintain Medicaid eligibility. Service coordinators are responsible for developing positive working relationships with participants and identified team members to ensure that needed ongoing support is provided. What You Bring Have at least three (3) or more years of experience in a social service or a healthcare-related setting have a bachelor's degree in Social Work, Psychology, or other related fields with practicum experience Register Nurse (RN) Ability to work independently and manage a dynamic caseload Strong interpersonal, organizational, and communication skills A valid driver's license and reliable transportation Who Thrives in This Role We've found that people who are: Empathetic and adaptable Collaborative and communicative Self-motivated and reliable Team players with a deep sense of accountability …tend to thrive in this role. This position is part of a collective bargaining unit and follows union-negotiated terms for compensation and benefits. Location & Schedule This position is based in Lehigh and Berks Counties, PA. Work is primarily community-based with some remote flexibility. Office hours are Monday through Friday, 8:30 AM to 4:30 PM. Serivce Coordinators have on-call responsibilities.
