People First Supports Coordination was established in 2012. We are caring and experienced professionals with many years of experience in the area of supports coordination services. PFSC staff are knowledgeable of a multitude of county resources and services within the Intellectual Disability and Autism System. We have an appreciation for the individual's needs, desires, choices, control and self determination. People First Supports Coordination believes in a team approach, working together with families and service providers to best support the individual. We help individuals achieve a higher standard of living and quality of life. We travel to see our consumers who live in Chester County, Montgomery County, Delaware county and surrounding areas . People First Supports Coordination is looking for Supports Coordinators to join our team. As a Supports Coordinator you will use your skills and knowledge to help individuals with intellectual disabilities on your caseload, alongside of their families and teams to ensure health, safety and happiness in all areas of the individuals' lives.
Job Responsibilities :
o SC meets with teams to develop a plan which reflects what is important to the individual and how the team will assist with meeting the Individual's goals. o SC updates plan as needed to reflect what is currently happening in the individuals' life. o SC coordinates services with paid providers and community resources to support the individuals needs. SC monitors the services to ensure they are meeting the individual's needs, being utilized according to authorization and staff are knowledgeable/following the service plan. o SC monitors individuals as according to ODP regulations to ensure the individuals are living Everyday Lives, are living in the least restrictive environment, are needing and receiving authorized services, are having all health and safety needs met. o SC communicates as needed with teams regarding any updates, concerns and issues and make sure they are addressed in a timely and appropriate manner. o SC follows timelines and guidelines as determined by ODP regulations and People First Supports Coordination, including documenting and keeping up with billing requirements. SC is required to have a valid drivers license and and insured/registered vehicle. We are looking for someone that is able to manage their time well and is very organized.

Full-time Pay:
$42,000.00 - $46,000.00 per year
