Available Benefits Help for . This position will be responsible for facilitating consumer activities on an assigned shift. Assisting consumers in training activities as specified in habilitation plans and activity schedule. Providing direct support to consumers. Assisting in everyday living skills and care of personal belongings. Completing data collections, observation notes and daily reports as instructed. Assisting consumers in transportation as needed. Must perform related duties as required or asked of by management. Promoting independence, choice and empowerment of consumers. We have 12 group homes (East, West, Lower Valley, NorthEast) and the Day Habilitation Program.
